The document is almost completely correct; there are just a few details that could probably be improved. "mouses" *IS* incorrect. "mice" *IS* correct. Ask Kensington, a company which makes keyboards, mice and trackballs. Also, it seems to me like the USB A connector is upside down, both the plug and the socket. You can determine what's up and down by looking at most USB sticks; they have a LED on the top-side. On many PCs, USB-accessories (such as keyboards), you'll find that the socket (eg. in a keyboard) has the "thin" side up; this makes the manufacturers of USB-sticks and DVB-T decoders put a LED on the side that is turned upwards when inserted into such keyboards/accessories/PCs. The reason is that there are many microcontrollers, that are quite slow, but just fast enough to support USB 1.0. The Atmel AVR is a good example. Some of them are 'born' without USB, and a USB driver can be made in firmware (see obdev.at). Other examples are cheap ICs, such as the FTDI and PL2303/PL2305 chips. I do not know if FTDI are still making USB 1.0 chips, but I think PL230x is still USB 1.0... So far every Phone I have checked ties the shield to their power ground. If the host also ties to power ground, and my PC does, then the shield becomes the main return. This surprised and confused me at 1st. Also the USB specification does not clearly state what to do with the shield, it states something along the lines of terminate in the customary way. Just thought this was interesting and passed it along. I think the phones take advantage of this when charging because it helps keep the voltage level up.
